Nestled in the leafy suburb of Fullarton, the Home provides quality residential aged care accommodation for 195 residents with 24 hour comprehensive care available that is based on shared visions and partnering, communication, best practice care and support with an attraction to achieve our potential at all times.
We proudly offer flexible care provision incorporating dementia specific care within a secure comfortable and home-like environs, palliative care, low density rehabilitation and risk managed clinical care to prevent residents from being transferred to hospital unnecessarily, and where the care can be administered at the Home.
Under the leadership and supervision of our qualified clinical team, our specially selected and trained care staff deliver excellence in care provision within a warm and friendly environment to ensure quality resident outcomes are achieved.
The Home has a committed and creative clinical care team that fosters better practice strategies to ensure that the overall effectiveness of resident outcome is directly related to the input of the care team along with the resident and their family.

Transition Care Program
The Home incorporates the Transition Care Program which provides time-limited, goal-oriented and therapy-focused packages of services to assist elderly people in the community to make the transition from a hospital stay to a lower level of care.
The emphasis of this type of care entails a low density program of rehabilitation with specific goal setting for achievable health and functional outcome.
To enter transition care they must have been assessed as eligible by an Aged Care Assessment Team (ACAT) while they are an in-patient of a hospital and they can only enter transition care directly after discharge from hospital.
Transition care is designed to improve independence and confidence after a hospital stay. It allows them to return home rather than prematurely enter residential care.
Palliative Care is caring for a loved one who has a terminal illness to which there is no anticipated cure.
The aim of Palliative Care is to maximize the quality of the resident’s life by facilitating a comfortable, peaceful and dignified death.
Treatment provided at Fullarton Lutheran Homes is in accordance with the Palliative Care Act and good palliative care principles.
The Home provides symptom management and care that is coordinated through the registered nurse and doctor. Palliative Care is a Team Approach where the resident is central to the care provided. To ensure the best possible quality of life it is sometimes necessary that other health professionals become involved with the resident’s care. This is based on individual needs of the resident and family.
At all times the Home encourages residents to remain within their familiar environment and cared for by familiar staff and surrounded by family.
